NB: Due to the fluid situation with COVID-19 vaccinations in pharmacy, a single appointment type is being set up with vaccine information, medical questions and vaccination consent containing the information required for both Pfizer and Janssen vaccines. The appointment name can be any name you choose, but Janssen is suggested initially. If you transition to Pfizer, please email me to modify the name and sub-title of the vaccine appointment type.
In the forms, please enter your pharmacy contact details, staff logins, vaccination slot duration, vaccination title and subtitle. Please also review and if required modify the patient information text, the vaccination consent text and the medical questions.
